Creating And Filling In Medical Inquiry Request

To add a Medical Inquiry Request, click the “+” button in the Requests section located on the left side of the Inquiry form.


After clicking the “+” button, a new medical inquiry form is created and ready to be filled in. We explore the different items and functions in the image below. 



  1. Requests Tabs: Each inquiry received will have its dedicated Request Tab 
  2. Add Request Button: The button allows the creation of additional requests 
  3. Medical Inquiry ID: Unique Identifier of the Medical Inquiry incorporates the Case ID and additional information specific to Medical Inquiry 
  4. Classification: Dropdown selection list with information to the inquiry such as on/off label, disease state etc 
  5. Related to Patient: Dropdown selection (Yes/No) on whether a patient is on the product 
  6. Category & Topics: Dropdown selection of a list of Categories and Topics related to the Medical Information question – relates to Metadata from the Product Module 
  7. Query Level: Dropdown selection which classifies the level of the query ie first, second, third line 
  8. Escalation Date: Field automatically populates when an escalation email has been sent out of En3 
  9. Request Fields (Original & Translated): Free text fields to include the question of the inquirer 
  10. Response Fields (Original & Translated): Free text fields to include the response to the inquirer 
  11. Response Delivery: Dropdown selection of how the response will be provided 
  12. Response Date: Field automatically populates when a response email has been sent out of En3 
  13. Verbal Response per Label: Tick box to include verbal response according to label if no document needs to be uploaded 
  14. Document Search Function: Free Text field which allows searching of the Document Library according to the metadata provided in the case 
  15. Upload File Area: Manually upload any documents which may need to be sent in the response to the inquirer
